The Center Invited to Play Role in Population Health Planning in Ohio

To help facilitate collaboration related to population health planning in Ohio, the Health Policy Institute of Ohio (HPIO) has been commissioned by the Governor’s Office of Health Transformation, the Ohio Department of Health, and Ohio Medicaid to develop recommendations for a Population Health Plan as part of Ohio’s CMS State Innovation Model (SIM) Initiative.

To accomplish this task, HPIO has formed several working groups including Ohio’s Population Health Infrastructure Subgroup, which will meet three times before the end of 2015. The Center for Health Affairs’ director of community outreach, Kirstin Craciun, has been invited to participate on this subgroup comprised of individuals representing hospitals, local health departments, the Governor’s Office of Health Transformation and the Ohio Department of Health.

The infrastructure subgroup is charged with two primary objectives to inform the SIM Population Health Plan:

  • Develop draft recommendations for development of a framework for state and community-level population health planning that:

    • Aligns state and community-level population health planning processes, priorities and objectives
    • Provides state and local/regional coordination for implementation of community-based health improvement activities
    • Identifies potential financing mechanisms for implementation of community-based health improvement activities
  • Develop draft recommendations for improving the State Health Assessment and State Health Improvement Plan 

A broader Population Health Planning Advisory Group has also been formed and tasked with aligning health priority areas, measures, objectives and evidence-based strategies with the design and implementation of the Patient-Centered Medical Home (PCMH) model.
