Community Health Program
The Center’s community health program empowers member hospitals to meet their community health goals and achieve equitable and improved health outcomes for all. To improve the health of the community, it is important to first understand the health status and health needs of Northeast Ohioans and the conditions that support or hinder health. Multiple forms of data are necessary, collaboration among cross-sector partners is critical and outreach to the community is essential.
Supporting hospitals in their community health work is central to The Center’s community health program. We seek to help our hospitals build strong partnerships and align their efforts with one another, public health departments, community stakeholders and community residents. We believe that by working together, whether through developing a collaborative Cuyahoga County CHNA, launching Healthy Northeast Ohio, or convening via the CHNA Roundtable, that we will have a greater collective impact.
