Fall Event Features Expert on Effective Transitions of Care



The Center for Health Affairs and Better Health Greater Cleveland will sponsor Transitions of Care - Improving Care Across Settings on Wednesday, November 2, 2011, from 7:30 a.m. to 1 p.m., at Cuyahoga Community College - Corporate College East. Click here to register for the event.

Keynote speaker, Eric A. Coleman, MD, MPH, Professor of Medicine in Health Care Policy and Research and Geriatric Medicine at the University of Colorado Denver, will identify key strategies for effective transitions of care, including: 

  • Building professional competency in care coordination.
  • Enhancing the role of patients and caregivers in improving the quality of their care.
  • Using health information technology to promote safe and effective care transitions.

Dr. Coleman also serves as Director of the University of Colorado Hospital’s Care Transitions Program, which is aimed at improving quality and safety during times of care.

This event, funded by Robert Wood Johnson Foundation through its Aligning Forces for Quality initiative, will feature a panel of regional providers moderated by Dr. Coleman. The panel will provide tactics they are using to reduce heart failure readmissions and improve the transitions of care. There is no cost to attend this event, but registration will be required.
