Register Now for Transitions of Care Event



Registration is now open for Transitions of Care – Improving Care Across Settings, an event sponsored by The Center for Health Affairs and Better Health Greater Cleveland. The conference, which is intended to support hospital efforts to reduce readmissions, takes place Wednesday, November 2, from 7:30 a.m. to 1 p.m., at Cuyahoga Community College - Corporate College East.

Keynote speaker, Eric A. Coleman, MD, MPH, Professor of Medicine in Health Care Policy and Research and Geriatric Medicine at the University of Colorado Denver, will identify key strategies for effective transitions of care, including: 

  • Building professional competency in care coordination.
  • Enhancing the role of patients and caregivers in improving the quality of their care.
  • Using health information technology to promote safe and effective care transitions.

This event, funded by Robert Wood Johnson Foundation through its Aligning Forces for Quality initiative, will feature a panel of regional providers moderated by Dr. Coleman. The panel will provide tactics they are using to reduce heart failure readmissions and improve the transitions of care. Click here now to register for this free event online.
