The Northeast Ohio Patient Navigation Collaborative, or NEOPNC, which was originally established in 2011 to promote patient navigation in the region, is aligning its efforts with The Center for Health Affairs’ strategic direction in 2015. While the group’s goal will still be to promote patient navigation in the region, it will do so by providing support to those involved with administering patient navigation programs in The Center’s member hospitals.
“We want administrators involved in this collaborative to share challenges, best practices and resources to advance patient navigation programs in Northeast Ohio’s healthcare organizations,” explained Carol Santalucia, vice president of patient experience for CHAMPS Healthcare and the leader of this initiative. “I’ve been lucky to be involved in this field for a very long time and I know there’s a lot we can learn from one another.”
The first meeting for the newly-envisioned group is scheduled for Tuesday, Feb. 10, from 8 a.m. to 9 a.m. at The Center’s offices. Additional meetings will be held quarterly. Patient navigation program administrators, as well as those interested in starting a program, are encouraged to attend and new members are welcome. For the first meeting, participants should be prepared to provide background information about their patient navigation programs and to share one success story and one challenge.
