On Friday, Oct. 10, 2014, The Northeast Ohio Patient Navigation Collaborative (NEOPNC), in conjunction with The Center for Health Affairs and the Academy of Medicine of Cleveland and Northern Ohio (AMCNO) convened a lay navigation event.
This lay navigation event, “The Critical Components of a Successful Navigation Program,” was featured in the AMCNO’s November/December publication of Northern Ohio Physician.
In the piece, "AMCNO Shares Insights on Creating a Successful Navigation Program at NEOPNC Event," CHAMPS Patient Experience’s Director of Operations Sarah Fay is quoted. “When building the navigation program, there are five important steps: create goals, assess and then break down barriers and challenges, choose how you want the program to operate (create a workflow), establish the role of the navigator, and create a physician champion to rally members of the care team to help make the program successful.”
