Kirstin Craciun, director of community outreach, participated on a Sept. 17, 2019 panel at Cleveland Clinic’s first Populations Health Symposium with:
- Merle Gordon, director of the Cleveland Department of Public Health
- Rita Horwitz, president and CEO of Better Health Partnership
- Dr. Claude Jones, chief medical officer of Care Alliance Health Center
- Panel Moderator: Dr. Nazleen Bharmal, medical director of community partnerships for Cleveland Clinic Community Care
Panel participants discussed how community partners and initiatives are being leveraged to improve health outcomes and equity in Northeast Ohio. Panel topics discussed at the Populations Health Symposium included the following:
- The aligned Cuyahoga County community health needs assessment (CHNA) process and involvement by many committed stakeholders.
- Importance of trust and how community health workers have been critical to helping build and sustain trust with members of the community.
- Prioritization process used to determine the health topics that will be the focus of a three-year implementation plan in Cuyahoga County.
