HealthComp Foundation Grant Awarded for Community Health Improvement



The HealthComp Foundation recently announced the latest funding awards granted in 2021, including a proactive grant to advance community health improvement and health equity in 2022. A grant of $170,000 has been awarded to the Cuyahoga County Board of Health. Grant efforts for 2022 will include a collaborative community health needs assessment with health departments and hospital systems, supporting coordinated implementation strategies and maintaining the healthyneo.org (Healthy Northeast Ohio) population health data platform. These efforts will be facilitated collaboratively by The Center for Health Affairs and the Cuyahoga County Board of Health.

 

The scope of the project, enhanced by the grant funding, will involve:

  1. The addition of new data sources, community health needs assessments, and community health improvement plans to healthyneo.org from regional participating partners, in addition to site maintenance and enhancements.
  2. Cuyahoga County Steering Committee collaboration to develop and complete a collaborative community health needs assessment and community health improvement plan.

In addition, those committing financial support in 2022 for the collaborative community health needs assessment and healthyneo.org efforts include: Cleveland Clinic, Cleveland Department of Public Health, Cuyahoga County Board of Health, The MetroHealth System, St. Vincent Charity Medical Center, Southwest General and University Hospitals. Additional regional partners committing financial support in 2022 to support healthyneo.org efforts include Ashtabula County Health Department, Lake County General Health District, Lorain County Public Health, Medina County Health Department and Summit County Public Health.
