The Center for Health Affairs just launched a regional social determinants of health (SDoH) survey to help shape the well-being of Northeast Ohio communities. Through a collaborative effort with community partners, the regional SDoH survey will identify the most pressing needs and concerns impacting residents. The data gathered will be the foundation for meaningful action—driving programs that enhance economic stability, social well-being, and health equity across the region.
Why Your Participation Matters
Social determinants of health influence everyone—where we live, learn, work, and play. Factors such as access to nutritious food, safe housing, reliable transportation, quality education, and a healthy environment all play a crucial role in determining health outcomes. By sharing your experiences and insights, you are helping to build a future where every individual and family in Northeast Ohio can thrive.
The survey focuses on 10 counties: Ashland, Ashtabula, Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, Summit, and Tuscarawas. Your input will guide the creation of impactful programs and policies tailored to the real needs of these communities.
What We Want to Learn
We are gathering insights on the following issues affecting everyone’s daily life:
- Physical and financial barriers to access healthcare.
- Meeting essential needs like food, clothing, shelter, and utilities.
- Financial stress related to housing and basic living expenses.
- The impact of climate change on household decisions.
- Transportation barriers that affect work, school, and health care access.

Our Commitment to Action
The Center for Health Affairs remains at the forefront of addressing health disparities. In 2022, we established the SDoH Innovation Hub, powered by Amazon Web Services. This helped us to tackle the root cause of health inequities through data-driven and community-led solutions. With more than 80 actively engaged organizations in Greater Cleveland, our first phase of work provided critical insights on behavioral health and firearm violence prevention.
